چکیده

BACKGROUND The early hemodynamic normalization of polytrauma patients may lead to better survival outcomes. The aim of this study was to assess the diagnostic quality of trauma and physiological scores from widely used scoring systems in polytrauma patients. METHODS In total, 770 patients with ISS > 16 who were admitted to a trauma center within the first 24 hours after injury were included in this retrospective study. The patients were subdivided into three groups: those who died on the day of admission, those who died within the first three days, and those who survived for longer than three days. ISS, NISS, APACHE II score, and prothrombin time were recorded at admission. RESULTS The descriptive statistics for early death in polytrauma patients who died on the day of admission, 1-3 days after admission, and > 3 days after admission were: ISS of 41.0, 34.0, and 29.0, respectively; NISS of 50.0, 50.0, and 41.0, respectively; APACHE II score of 30.0, 25.0, and 15.0, respectively; and prothrombin time of 37.0%, 56.0%, and 84%, respectively. These data indicate that prothrombin time (AUC: 0.89) and APACHE II (AUC: 0.88) have the greatest prognostic utility for early death. CONCLUSION The estimated densities of the scores may suggest a direction for resuscitative procedures in polytrauma patients. Independent Predictors of Early Death of Polytrauma Patients: An Analysis of 696 Patients

Polytrauma patients are at high risk to die due to acute physiological deterioration after a high injury load within the first hours. Trauma scores such as ISS, NISS and APACHE II and laboratory parameters were collected at admission. The aim of this study was to evaluate the independent predictors of early death of the polytrauma patient. A total of 696 patients (age 40.8 ± 15.5 years) who had...

The Utility of Trauma Scoring Systems in Outcome Prediction of Critical Patients

Background: Mortality predictions calculated using scoring scales are often not accurate in populations other than those in which the scales were developed because of differences in case-mix. Scoring systems should be validated to be used in a different population. Methods: Probabilities of ICU death for critically ill trauma patients were calculated using the anatomic scores: Injury severity s...

Comparison of four prognostic markers in opioid poisoned patients admitted in ICU

Background: Opioid poisoning is the most common type of poisoning in intensive care units (ICUs). This group usually includes patients who have been drug abusers for a long time and now require hospitalization either because of acute overdose or due to side effects of routine opioid use. This study aimed to compare the severity and prognosis of patients using common mortality predictors Sequent...

Association between admission hypomagnesemia mortality or mortality of critically ill patients in intensive care unit

Background: Up to now there is no study evaluating correlation between serum magnesium and morbidity or mortality in patients admitted in intensive care unit. The aim of this study is to determine the prevalence of hypomagnesemia in critically ill patients and to evaluate its association with organ dysfunction, hospitalization period and mortality. Methods: We conducted a retrospective trial ...

The Quality Assessment of Performance in Intensive Care Units According to APACHE II Score

Introduction: Improving the health level of patients is one of the most important purposes in intensive care units. In order to promote these units we need to measure their quality. To do so, some standards are needed in this area. The aim of this article was to study the mortality rate of the patients admitted to intensive care units with different APACHE scores. <strong...
